Cancellation Policy

1. Acknowledgment of Risks: I acknowledge that participation in basketball activities, including but not limited to practices, games, and related events, involves inherent risks of injury. I understand that such risks can include, but are not limited to, physical injury, emotional stress, and even death. I acknowledge that these risks are part of the nature of basketball and agree to assume all such risks associated with my child's participation. 2. Release of Liability: In consideration of my child's participation in the Arch City Select Basketball Club Program, I hereby release, discharge, and hold harmless Saint Louis Basketball, its officers, directors, coaches, employees, volunteers, and agents (collectively, "Released Parties") from any and all claims, demands, actions, or causes of action, whether known or unknown, arising out of or related to any injury, illness, or other harm that may occur as a result of my child's participation in the program, including but not limited to negligence on the part of the Released Parties. 3. Medical Treatment Authorization: I authorize the staff of Arch City Select Basketball Club to administer first aid and seek medical treatment for my child if necessary. I agree to be responsible for any medical expenses incurred as a result of such treatment. I also acknowledge that Arch City Select Basketball Club does not provide medical insurance for participants. 4. Photo and Media Release: I consent to the use of my child's photograph, video, or likeness by Arch City Select Basketball Club for promotional or informational purposes, including but not limited to social media, websites, and printed materials. I understand that no compensation will be provided for such use. 5. Understanding and Agreement: I have read and fully understand this Waiver and Release of Liability. I understand that by signing this form, I am giving up substantial rights, including the right to sue. I acknowledge that I am signing this form voluntarily and with full knowledge of its significance.
